Job Summary: The Housing Navigator will assist clients in identifying, securing, and maintaining stable housing. This role involves working closely with individuals to assess housing needs, provide guidance on available housing options, and support clients through the application and leasing process. The Housing Navigator will also collaborate with landlords, housing authorities, and service providers to ensure clients have access to necessary housing resources and support. 

Job Title: COMMUNITY HEALTH WORKER

Job Summary: The Community Health Worker (CHW) will serve as a liaison between healthcare providers, social services, and community members to ensure individuals receive the care and resources they need. The CHW will work directly with clients to provide health education, support access to services, and promote healthier lifestyles. This role is critical in improving health outcomes by bridging gaps in care, addressing social determinants of health, and empowering individuals to manage their health. *Applicant must already possess CHW credentials*
